The Madison County Health Department is now conducting a Health and Safety online survey.
The Madison County Health and Safety Survey, the starting point for data gathering and input for MCHD’s 2020-2025 community health assessment (CHA) cycle, is now live.
This survey, a partnership with faculty and students from EKU’s Public Health program, can be accessed online in English and in Spanish.
The Health Department says that it is vital to hear from the community and would like every sector and population group of adults to be represented in this survey.
Paper copies of the survey will also be available at the Madison County Health Department clinics in Richmond and Berea.
